What you'll do

• Evaluate and manage patients with serious, chronic, or life-limiting illnesses
• provide symptom management
• conduct goals-of-care discussions, advance care planning, and family meetings
• provide inpatient consultation and/or outpatient clinic services
• collaborate with care teams
• support care coordination and transitions, including hospice referral when appropriate
• participate in interdisciplinary rounds
• educate patients, families, and teams
• document care
• participate in quality and safety initiatives
• supervise and educate trainees as applicable.

Why this role
