Community Resource Advocate

Job overview

The Community Resource Advocate role is in person in Asheville, NC, and pays $17–$20 per hour. Appalachian Mountain Health provides primary and preventive care to medically underserved and uninsured populations in western North Carolina. The advocate connects patients with public benefits, community resources, and care services to support stability and self-sufficiency. The role coordinates with clinic teammates and community partners so patients can navigate care and access needed supports.

What you'll do

• Coordinate access to Medicaid applications, financial services, shelter, transportation, food resources, behavioral health, dental care, and other supports
• identify barriers before appointments
• maintain patient-specific medical-record documentation and an up-to-date community resource database
• maintain community partnerships
• support a caseload of at least six patient encounters per day.
