Job overview

The Social Work Case Manager role is onsite in Chico, CA, with work from the Chico office and travel to clients’ homes in Butte and surrounding counties. Pay is $26–$33 per hour. Home & Health Care Management provides in-home care and case management for older adults, children, and people with disabilities.

What you'll do

• Help participants access community resources and services
• advocate for timely medical care
• coordinate with the Care Management Team, participants, and other entities
• educate participants about benefits and supports
• assist with applications
• document interactions and report significant changes
• and participate in daily case conferences.
