The Registered Nurse Assessment Coordinator, RNAC/MDS Coordinator role is onsite in Keosauqua, IA. Compensation is $40–$45 per hour. The full-time position supports Country Lane Manor’s skilled nursing team and resident-centered care. The role includes an on-call rotation every three to four weeks, including weekend on-call responsibilities. The coordinator completes resident assessments and MDS documentation, develops individualized care plans, and works with the interdisciplinary team to support regulatory compliance and quality care. The role helps ensure resident needs and changes in condition are accurately assessed and communicated.

What you'll do

• Conduct routine and change-related resident assessments
• observe and communicate changes in condition
• collaborate with Social Services, Activities, Dietary, Therapy, and Nursing
• complete MDS assessments, quarterly reviews, CAAs, and care plans
• distribute the MDS schedule
• transmit MDS data to the state and maintain records
• participate in the rotating on-call schedule.
