Discovery at Home provides Part A, Part B, and private-duty care, primarily serving residents in communities managed by Discovery Senior Living. The CNA supports clients’ daily care and well-being through personal assistance, companionship, mobility support, meal preparation, light housekeeping, and timely care documentation. The role contributes to safe, consistent care in clients’ homes and during outings or appointments.

What you'll do

• Assist clients with bathing, grooming, dressing, eating, mobility, and transfers
• provide companionship and emotional support
• administer medications as directed by healthcare professionals
• prepare meals according to dietary needs and preferences
• perform light housekeeping
• accompany clients to appointments and outings as needed
• document care provided.
