Following care plans and communicating changes to the Home Care Manager helps support each client’s physical, social, and emotional well-being.

What you'll do

• Provide personal care and companionship

• provide light housekeeping and meal preparation and transportation as appropriate
• follow care plans and instructions
• communicate changes and concerns to the Sage Home Care Manager
• complete an electronic daily shift report
• comply with policies and applicable laws and regulations.
