What you'll do
- Monitor host family homes through twice-monthly visits
- coordinate with the Community Support Team
- maintain and review medication records, goal progress, safety drills, and case notes
- support members' independent functioning and community integration
- implement individual or group independent-living programs
- monitor home safety and cleanliness
- assist with medication according to program guidelines and required authorization
- respond to medical or behavioral emergencies and report incidents
- accompany or transport members to appointments and services as needed.
