What you'll do

• Remain available by telephone and email during assigned shifts
• conduct urgent physical and psychosocial assessments
• provide nursing care, pain control, and symptom management
• guide patients and families on diagnoses, medications, treatments, and progress
• document care and changes
• collaborate with physicians, the interdisciplinary group, vendors, hospitals, and community residential staff
• conduct after-hours hospice admissions as necessary
• attend required meetings and continuing education.
