What you'll do

• Supervise reentry care coordinators and support patient-centered needs assessments and care plans
• manage the RISE patient panel and caseload assignments
• oversee referrals and transition plans
• coordinate communication and jail visits across teams
• monitor quality metrics and documentation
• support reporting and compliance requirements
• co-facilitate care coordination, overdose prevention, and recovery groups
• lead weekly team huddles
• maintain reentry resources and community contacts.
