Intensive Recovery Support Coordinator II

Park Center’s Permanent Supportive Housing team provides recovery-oriented, housing-focused intensive case management. Through person-centered tenancy support, recovery coaching, community connections, and practical skill-building, the role helps members pursue personally meaningful goals and sustainable housing.

What you'll do

• Support residents transitioning to independent housing
• build trust using trauma-informed and culturally humble engagement
• provide strengths-based tenancy support
• help members pursue personal goals and whole-health recovery
• coach independent living skills
• connect members with community resources
• encourage employment, volunteering, and social or recreational participation
• support interpersonal skills and natural supports
• maintain required documentation
• participate in the team’s on-call rotation and after-hours responsibilities.
