What you'll do

• Conduct intake and individualized needs assessments
• collaborate on Individualized Service Plans addressing housing, income, health, and other goals
• provide referrals and linkages
• complete program assessments
• review goals with participants
• maintain accurate case files and confidentiality
• build professional relationships
• provide crisis intervention and risk assessment
• use motivational interviewing and trauma-informed care
• maintain knowledge of community resources.
