Registered Nurse (RN) Care Manager and Patient Advocate

The Registered Nurse (RN) Care Manager and Patient Advocate role is onsite in Cincinnati, Ohio. Compensation is $40–$45 per hour, with bonus opportunities. Trinity Aging Life Advisors provides life care management to seniors and helps clients and families navigate aging and healthcare challenges while promoting independence, health, and safety.

What you'll do

• Build relationships with older adult clients and their families
• conduct in-home assessments and ongoing care evaluations
• coordinate care and communicate with healthcare providers
• attend medical appointments as an advocate
• monitor medications and treatment plans
• help clients access healthcare and community resources
• collaborate with home health and caregiving teams
• and serve as a primary contact for clients and families.
