The Outreach and Community Engagement Coordinator works onsite with the Arizona team, with the listing location in Winslow, AZ. The role pays $18–$27 per hour. Soaring Eagles Home Care serves seniors, children, and people with disabilities, with a mission to honor those it serves and help clients remain independent at home and in their communities. The coordinator builds culturally respectful community relationships and provides education about Medicaid/AHCCCS and ALTCS programs. By supporting eligibility awareness, enrollment, referrals, and communication between community members and providers, the role helps people access appropriate care and supports smooth transitions into services.

What you'll do

• Build relationships with Navajo and Hopi community members and local organizations
• conduct outreach at community events, provider sites, chapter houses, and other venues
• provide culturally sensitive education about Medicaid/AHCCCS and ALTCS eligibility and enrollment
• deliver presentations and distribute information
• maintain partnerships with healthcare and referral organizations
• serve as a provider contact and liaison
• support referral workflows and transitions into care
• track outreach, contacts, events, and referral data
• report outreach outcomes, community needs, and trends.
