Hospice Registered Nurse (RN) Case Manager, Per Diem

The Hospice Registered Nurse (RN) Case Manager, Per Diem role is onsite in Apple Valley, CA, serving patients in their homes or residential facilities throughout Los Angeles County. Compensation is $75–$100 per hour. Angel Care Hospice & Palliative provides end-of-life care and support for patients, families, and caregivers. The RN develops and manages individualized plans of care and coordinates with an interdisciplinary hospice team. The role contributes to effective, compassionate care by assessing patient needs, administering and monitoring medications, documenting changes, and communicating with colleagues.

What you'll do

• Coordinate with colleagues to assess, plan, implement, and evaluate patient care plans
• communicate patient needs to the hospice team
• administer medications and monitor for side effects or reactions
• document and report changes in patient condition
• maintain accurate care records
• discuss observations with a supervisor
• and follow professional standards, protocols, rules, and regulations.
