Community Habilitation Specialist

The Community Habilitation Specialist role is onsite in Troy, NY, with compensation of $18–$21 per hour. The specialist works one-to-one or in small groups to help individuals build or maintain skills for safer, more independent community living. The role supports community participation, social connections, self-advocacy, and person-centered services while documenting progress and meeting billing requirements.

What you'll do

• Promote safety, independence, and learning
• support community connections, social participation, choice-making, and self-advocacy
• provide person-centered services in community settings
• complete required training and documentation
• attend Life Plan meetings as needed
• create schedules based on individual preferences
• meet weekly hourly billing requirements.
