MDS Coordinator – Skilled Nursing Facility

The MDS Coordinator – Skilled Nursing Facility role is onsite in Stockton, CA. Compensation is $43–$45 per hour. The coordinator supports resident assessment and care planning at Skilled Nursing of California. The role manages the Minimum Data Set and Resident Assessment Instrument process to ensure accurate assessments, regulatory compliance, and appropriate reimbursement under PDPM. It coordinates clinical information across departments and supports individualized resident care plans.

What you'll do

• Complete and coordinate MDS assessments under CMS RAI guidelines
• ensure accurate assessments, care plans, and supporting documentation
• conduct resident interviews
• validate clinical information with nursing, therapy, and social services
• ensure accurate diagnosis coding and PDPM classification
• review documentation supporting skilled services and reimbursement
• develop and update individualized care plans
• lead interdisciplinary care plan meetings
• maintain regulatory compliance
• assist with surveys, audits, and quality reviews
• participate in QAPI
• educate staff on documentation standards.
