Admissions Coordinator

The coordinator evaluates prospective admissions and helps direct patients to an appropriate center, while coordinating information and communication among referral sources, patients, and interdisciplinary teams. The work supports smooth transitions into the communities and helps connect people with services that meet their care needs.

What you'll do

• Complete daily rounds in designated hospitals
• obtain and assess high-acuity inquiries and special patient needs
• evaluate inquiries and communicate decisions to case managers and admissions staff
• facilitate patient transitions
• gather discharge-plan information from physicians, patients, and referral sources
• develop relationships with hospital and community referral partners.
