The Transitional Assistant Coordinator — Essex Area role is onsite in Lynnfield, MA, and requires travel using a reliable, insured vehicle. Compensation is $25–$35 per hour. Guardian Angel Senior Services provides home care and supports adults with developmental disabilities, people who are blind or visually impaired, and adolescents with brain injury. The coordinator helps participants transition from skilled nursing facilities to community living with appropriate supports. The role advances participants’ independence and safety by coordinating housing, transition planning, essential household setup, service documentation, and communication with case managers and discharge planners.

What you'll do

• Assist with housing searches and applications
• attend housing appointments and transition meetings
• help set up apartments and utilities
• communicate with case managers
• document services, applications, expenses, and participant communications
• assess housing needs
• purchase essential furnishings and household items based on participant preferences
• submit activity logs, delivery reports, and reimbursements.
