Habitat Health provides coordinated medical and social care through its Program of All-Inclusive Care for the Elderly (PACE), supporting older adults in centers and at home. The assistant supports participant-centered care by completing intake, rooming, routine clinical procedures, documentation, and follow-up coordination. The role works with providers and the Interdisciplinary Team to support safe, efficient care and communicate changes in participant status.

What you'll do

• Complete intake and room participants
• obtain vital signs and screenings
• fulfill provider orders and prepare participants for examinations and treatments
• perform laboratory testing, phlebotomy, EKGs, immunizations, injections, and other routine procedures within scope
• coordinate follow-up visits and assessments
• respond to participant questions
• maintain clinical documentation
• support referrals, records, authorizations, clinic readiness, and inventory.
