The role supports outreach, referrals, warm handoffs, and follow-up with daytime teams and community partners.

What you'll do

• Assess health and social needs of people experiencing homelessness
• conduct street and subway outreach in Brooklyn and Queens
• coordinate referrals to overnight facilities
• accompany and advocate for participants
• support warm handoffs and follow-ups with SOS day teams
• maintain relationships with community partners.
