Partner with cross-functional teams and senior leaders to deliver innovative and high-quality care in the home and community for high-need, high-risk, high-utilizing patients. Provide direct patient care (approximately 20% of time). Provide clinical oversight to the members of the Interdisciplinary Care Team. Lead and manage clinical teams, providing oversight, mentorship, and direction.
