What you'll do

• Conduct intake and individualized needs assessments
• collaborate with participants on Individualized Service Plans
• provide referrals, linkages, information, and support
• complete program assessments
• review progress toward short- and long-term goals
• maintain timely and accurate case files and confidentiality
• build professional relationships with clients and partners
• provide crisis intervention and risk assessment using motivational interviewing and trauma-informed care
• maintain knowledge of community resources.
