What you'll do

• Provide personalized case management, intake, assessments, and service plans
• connect participants with housing, employment, health, and other community resources
• facilitate workshops
• welcome families and develop plans
• build community service networks
• maintain records and reports
• participate in meetings, training, and community events
• support compliance and attend hearings as needed
• encourage participant involvement.
