MDS Coordinator

What you'll do

• Assess new admissions and determine health status and level of care
• complete and track MDS assessments within regulatory time frames
• coordinate interdisciplinary participation and ensure documentation is complete
• enter and submit MDS data electronically and verify submissions
• schedule and conduct resident care conferences
• assist disciplines with formulating and revising care plans
• evaluate care plans and resident outcomes
• coordinate level-of-care changes and required notifications and forms.
