What you'll do

• Conduct outreach and eligibility and enrollment assessments
• engage patients and facilitate care management where accessible
• complete risk assessments and develop patient-centered care plans
• monitor plan implementation and treatment adherence
• educate patients on self-management
• use motivational interviewing and trauma-informed care
• participate in case conferences
• connect patients to social services
• coordinate discharge planning
• accompany patients to visits as needed
• maintain monthly in-person contact and document encounters in the EHR.
