What you'll do

• Plan, implement, and evaluate the Social Services program
• prepare for surveys and inspections
• complete social histories, psychosocial assessments, and relevant MDS sections
• participate in care planning
• provide therapeutic adjustment support
• educate residents and families
• coordinate discharge planning and referrals
• maintain documentation and community agency relationships
• participate in quality assurance and team meetings
• report suspected abuse, neglect, or exploitation.
