What you'll do

• Conduct home admission assessments
• develop individualized care plans
• complete IHSS supervision visits and documentation
• monitor client progress and reassess needs
• train and support caregivers
• supervise IHSS attendants
• collaborate with clients, families, case managers, physicians, and agency staff
• maintain compliant records
• travel to client homes.
