What you'll do

• Manage day-to-day quality reporting for Medicare, ACO REACH, and commercial programs
• distribute and monitor gaps-in-care reports
• support data collection, validation, and analysis
• train providers and staff on program requirements, documentation, and workflows
• support CAHPS survey readiness and response-rate strategies
• participate in payer audits and committee meetings
• collaborate with practices on diabetes, hypertension, and preventive-screening outcomes.
