What you'll do

• Conduct client intakes and assessments
• coordinate community transitions with multidisciplinary teams
• help clients understand leases and program agreements
• serve as a contact for landlords and property managers
• respond to housing emergencies and follow up on maintenance requests
• assess accommodation and unit modification needs
• educate and advocate with landlords and community members
• research affordable housing
• assist with applications and tenant conflict mediation
• maintain the housing registry and confidential records
• attend meetings as needed.
