What you'll do

• Work with Community Health Advocates to address social needs of Food Rx patients.
• Collaborate with outpatient clinic providers to identify eligible patients and coordinate enrollment.
• Provide diabetes and cardiovascular education, nutrition counseling, and weight management strategies.
• Report to the Regional Clinical Manager for Food Rx.
• Incorporate evidence-based strategies into population health management for patients with complex medical and social needs.
