Hospice Social Worker, Part-Time

The Hospice Social Worker role is onsite in Uniontown, OH, serving patients in Summit and Medina counties. The position is 20 hours per week, Monday–Friday, 8:00 a.m.–4:30 p.m. Compensation is a competitive per-visit rate, with no numeric rate specified. The role coordinates with the interdisciplinary group, supports coping and resource access, documents care, and helps the team respond to changing patient and caregiver needs.

What you'll do

• Complete psychosocial assessments within five days of hospice admission
• participate in interdisciplinary group meetings and care planning
• observe and document emotional and social changes
• maintain clinical records
• evaluate patient and family responses to social work interventions
• coordinate visit schedules with case managers
• participate in quality assurance activities and discharge planning as needed
• assess family coping with the patient’s dying
• consult with the health team
• connect patients with family and community resources.
