What you'll do

• Guide treatment plans with the interdisciplinary team
• coordinate tentative discharge and contingency plans
• participate in discharge planning
• facilitate weekly team conferences and treatment-plan changes
• complete case-management documentation
• review reports and payer requirements
• meet with patients and families within required admission timeframes
• educate them about rehabilitation and the Case Manager role
• coordinate referrals and resources
• participate in utilization review and the on-call schedule as needed.
