PRN Registered Nurse Case Manager

What you'll do

• Complete initial and ongoing patient and family assessments
• provide professional nursing care
• develop and revise plans of care
• administer prescribed medications and treatments
• counsel and educate patients and families
• coordinate assigned caseloads when designated as case manager
• maintain clinical notes
• communicate changes and needs to physicians and community health partners
• participate in interdisciplinary meetings and quality improvement activities
• participate in on-call duties as defined by policy.
