Bilingual Field Care Coordinator / Field Case Management

The Bilingual Field Care Coordinator / Field Case Management role is field-based in Orange County, California, with one work-from-home day per week and regular local travel. Pay is $30 per hour. Pair Team’s care model coordinates clinical, behavioral, and social support for high-need patients. The coordinator builds relationships with members in their homes and communities, connects them with care and social resources, and supports continuity through individualized care plans, follow-up, and collaboration with the multidisciplinary care team.

What you'll do

• Serve as the primary contact for members with complex needs
• conduct in-person onboardings and visits
• deliver care packages and collect health readings such as blood pressure
• coordinate appointments, services, and referrals
• collaborate with nurses and behavioral health care managers
• address barriers to care
• maintain individualized care plans and timely documentation
• advocate for members
• and participate in care team meetings.
