What you'll do

• Use motivational interviewing to engage clients
• build and maintain a network of community service providers
• meet with clients and transition case managers before release to review plans and connect them with services
• coordinate with government program administrators and community resources
• support access to eligible benefits and identification
• schedule post-release follow-up calls and monitor progress
• conduct provider site visits
• compile reporting data
• and serve as a liaison among clients, providers, and facilities.
