Mom’s Meals provides home-delivered tailored nutrition solutions for people managing chronic conditions and other health needs. The contractor provides member-centered nutrition assessments and counseling for Medically Tailored Meals programs. By evaluating nutrition needs, recommending suitable meal options, documenting interactions, and connecting members with resources, the role supports members’ health and program access.

What you'll do

• Provide telephonic nutrition assessments and counseling using clinical protocols and independent professional judgment
• assess meal appropriateness and program eligibility
• review intake, allergies, preferences, and swallowing concerns
• recommend meal types and provide nutrition education
• connect members with resources
• document interactions within one business day
• maintain required RDN credentials, licenses, and telehealth authority
• escalate urgent clinical and safety concerns
• protect member information through approved systems.
