What you'll do

• Complete comprehensive assessments and develop Life Plans with individuals and their support teams
• conduct face-to-face visits as required
• coordinate care and health promotion, appointments, referrals, and transitions between levels of care
• support individuals and families and maintain monthly contact
• use health information technology to coordinate services
• document service delivery in the electronic health record while following privacy requirements
• assist with benefits maintenance and report abuse, neglect, or maltreatment immediately.
