What you'll do

• Supervise, coach, and train three Case Managers
• coordinate with Lilyfield case management staff
• monitor caseloads and service quality
• oversee intake, assessment, goal setting, implementation, and evaluation
• review Family Service Plans and SMART goals
• monitor case documentation and data
• coordinate community referrals
• lead case reviews and crisis response
• analyze quarterly impact data and prepare reports
• support resident engagement and continuous quality improvement
• ensure policy, funder, and confidentiality compliance.
