The professional supports clients’ daily living, independence, safety, and community participation while implementing individualized care plans. The role works with clients, families, managers, and providers to support personal goals and meaningful daily activities.

What you'll do

• Assist with activities of daily living, including bathing, dressing, grooming, toileting, and meal preparation
• administer medications, monitor vital signs, and support medical appointments
• maintain a clean and safe living environment
• engage clients in social activities and community outings
• use mobility equipment with proper techniques
• document activities and incidents
• maintain confidentiality
• implement individualized care plans.
