What you'll do

• Collaborate with physicians, social workers, chaplains, and nursing assistants on patient care
• prepare and adjust care plans
• visit patients to monitor vital signs and comfort
• perform prescribed treatments and administer pain management medications
• educate and support loved ones and caregivers
• maintain medical charts and health records
• notify physicians, hospice staff, and family members of patient deaths
• advise grieving family members about coping techniques, counseling, and community resources.
