What you'll do
- Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
- Conduct initial and ongoing assessments.
- Support the IDG process through chart reviews and interdisciplinary communication.
- Educate patients and families on disease progression and symptom management.
