The Clinical Case Manager, SAGE at Clover Place role is onsite in Seattle, WA, with compensation of $39.08–$43.15 per hour. DESC’s SAGE program provides outpatient mental health services and long-term case management to adults with severe and persistent mental illness. The case manager coordinates ongoing support, housing retention, and access to clinical and community resources. Through outreach, assessment, crisis response, advocacy, and collaboration with housing and care teams, the role supports clients’ recovery, stability, independence, and self-sufficiency.

What you'll do

• Provide ongoing case management and outreach
• collaborate on client care and housing retention plans
• conduct psychosocial assessments and develop goal planners with clients
• coordinate psychiatric and substance use treatment and access to food, clothing, housing, medical care, financial assistance, and vocational resources
• advocate for clients and coordinate with community providers
• make local home visits and support housing retention
• provide crisis intervention and risk assessment
• document services and maintain records.
