The Outreach Coordinator role is onsite in East Syracuse, NY. Pay is $28.85 per hour. The position supports Venture Forthe by connecting prospective clients with medical and social services and building community referral relationships. The role develops referral partnerships, meets with individuals and families, coordinates referrals and documentation, and represents the organization in community settings. Regular travel throughout the assigned service area is expected, up to 80%.

What you'll do

• Develop relationships with hospitals, physician offices, nursing homes, community organizations, and other referral sources
• meet prospective clients and families in community and home-based settings
• coordinate medical and social-service referrals and documentation
• respond to referral partner questions
• represent Venture Forthe at community events and meetings
• maintain outreach records in CRM and data-tracking systems
• protect confidential information
• travel regularly throughout the assigned service area, up to 80%.
