What you'll do

• Perform initial nursing assessments for new admissions
• complete, coordinate, and revise care plans
• communicate patient progress to the clinical manager, physician, and care team
• maintain patient progress records
• document pain and symptom management changes and outcomes
• educate families
• make referrals to other hospice disciplines as needed
• perform assigned on-call responsibilities and services.
