Participant Experience Liaison

WelbeHealth’s PACE program provides all-inclusive medical and community-based services to help seniors remain in their homes and communities. The liaison connects participants with the PACE system of care and serves as a communication link between participants and their care team. The role supports onboarding, appointment satisfaction, grievance resolution, and ongoing participant check-ins.

What you'll do

• Communicate with interdisciplinary team members and external providers
• support participant experience projects and satisfaction surveys
• facilitate communication among providers, families, participants, and caregivers
• make customer service calls about appointments
• support grievance resolution
• attend required meetings
• check in with participants about concerns and disenrollment risk.
