What you'll do

• Screen and assess members and families
• develop individualized housing support plans
• coordinate with members, interdisciplinary teams, employers, and training programs
• provide culturally competent case management and resource matching
• support life skills, job preparation, home care, and financial literacy
• maintain records and progress reports
• liaise between landlords and tenants
• conduct post-housing home visits
• coordinate case conferences and monitor progress toward goals.
