Psychiatric Rehabilitation Specialist (CCBHC)

The Psychiatric Rehabilitation Specialist (CCBHC) role is onsite in Buffalo, NY, with regular local travel; the schedule is Monday through Friday day shifts, with some flexibility for evenings as needed. Compensation is $20–$24 per hour. The role supports Horizon Health Services’ Open Access team and community behavioral health services. The specialist helps individuals pursue recovery, wellness, independence, and community integration through rehabilitation support, skill building, and connections to clinical, educational, vocational, and community resources. The role coordinates with providers and partners to support person-centered plans and sustained engagement.

What you'll do

• Provide rehabilitation support through outreach calls, telehealth, and face-to-face interactions
• help individuals identify recovery barriers and build practical skills
• conduct initial needs assessments and connect people to appropriate services
• support educational and vocational goals
• coordinate community linkages and follow up on progress
• participate in multidisciplinary case conferences and treatment team meetings.

Why this role

Requires a bachelor’s degree and six months of relevant experience, an associate degree and one year of relevant experience, or a high school diploma/equivalent and two years of relevant experience. Requires a valid New York State driver’s license and reliable transportation. Grant funded.
