What you'll do

• Conduct screenings and assessments using validated clinical tools
• provide individual counseling and therapeutic support in community and residential settings
• develop and update individualized treatment plans and measurable goals
• coordinate clinical information with multidisciplinary team members
• support crisis assessment and intervention
• complete clinical documentation and participate in quality improvement and required training.
