Foster Youth Navigator (Chafee Program)

Foster Youth Navigator (Chafee Program) is a hybrid role based in Fort Collins, CO, with hybrid work available for up to 20% of total work hours. Pay is $22–$23 per hour. The Matthews House works to empower youth and families through connection, self-efficacy, and independence. The Navigator helps youth preparing to age out of foster care access Chafee-funded services and supportive relationships. Through coaching, case management, advocacy, and coordination across community systems, the role helps reduce barriers and advance goals in education, employment, housing, and independent living.

What you'll do

• Provide coaching and case management to Chafee youth, including independent-living planning
• maintain client contact, conduct intakes, and develop and revise case plans
• help youth navigate independent-living, education, employment-readiness, and housing resources
• accompany clients to appointments and other essential engagements when appropriate
• document services and maintain confidential files
• coordinate referrals and services with community partners.

Why this role

Hybrid option is limited to up to 20% of total work hours. Schedule is generally Monday–Friday, 9 a.m.–5 p.m., with occasional evenings more frequently required to meet with youth. Associate degree in Criminal Justice or Social Work is required.
