What you'll do

• Develop relationships with hospitals, rehabilitation and skilled nursing facilities, senior living communities, physician practices, and community partners
• follow targeted outreach routes
• identify referral decision-makers
• conduct needs-focused conversations
• schedule follow-up meetings
• maintain partner contact
• represent the agency at events
• document visits and next steps
• communicate qualified opportunities
• and follow HIPAA and compliance requirements.
